It's been a while since I really dug into this, but I took a quick look on
my iPhone.
I didn't try reading a Word document on iPhone. I've had issues with
VoiceOver and the Word app in the past, and I would expect some
issues trying to open a Word file in Pages to read math content.

VoiceOver can read MathML content from a web page, but it has errors. That
matches my past experiences. Increasing your Verbosity settings to include
"All" punctuation seems to help a bit. VoiceOver was not announcing plus
and equals in a simple problem until I changed the punctuation settings.
Even after that adjustment, VoiceOver seems to read grouping symbols, like
parentheses, inconsistently. Sometimes it's reading them perfectly,
sometimes it's skipping them entirely (including omitting the symbols from
the braille output).
VoiceOver seems to be reading all of the MathJax examples
<https://www.mathjax.org/#samples> correctly and consistently. Braille
output looks good too, from my quick check.
I think an HTML file with MathJax would probably be your best bet for
accessing this content with VoiceOver.
