[UDSM Leadership] Updated data collection forms & Slack Invite

Hi everyone,

My apologies for the delay in getting back to you all! I had an unexpected emergency come up this last week.

Please let me know your thoughts on how this form is set up and any recommendations you may have that will help you use it with more ease! I will also be reaching out to everyone individually this week as well.

Regarding slack: I've attached the slack join link below. Please join as soon as you are able to, but before Friday July 15th. I will be hosting some slack intro sessions next week where people can ask questions and work through navigating the slack channel with me.
