[UDSM Leadership] Leadership Transition Tool

Hi all,

I know that UDSM has a bunch of leadership transitions coming up.  Please utilize the attached leadership transition checklist as a guide.  I realize that this comes from a SOM perspective, please edit as you see fit.

Also, once this is filled out, please send the completed version to myself so that we can keep this on file from year to year.
